What You'll Do

As an LPN at CCHS, you'll work closely with providers and other members of the clinical team to support safe, efficient, and coordinated patient care. Your responsibilities will include:

• Performing nursing assessments and obtaining vital signs, health histories, and other clinical information
• Performing routine tests and screenings and communicating findings or concerns to providers
• Administering medications, treatments, injections, and immunizations as ordered
• Assisting providers with patient examinations and procedures
• Triaging patient calls and visits within the LPN scope of practice
• Providing patients and caregivers with education regarding medications, treatment plans, preventive care, and other healthcare needs
• Coordinating specialist referrals, testing, procedures, appointments, and prior authorizations
• Supporting patients with complex healthcare needs through care coordination and care management activities
• Maintaining accurate and complete clinical documentation
• Reviewing immunization status and administering and documenting immunizations
• Collaborating with providers, clinical staff, patients, caregivers, and outside healthcare facilities to support continuity of care
• Participating in Quality Improvement activities and supporting CCHS clinical standards and protocols

What We're Looking For

• Degree or diploma in Practical Nursing from an accredited nursing program
• Current and valid Licensed Practical Nurse (LPN) license in the state of Georgia
• Current BLS/CPR certification
• One to three years of nursing experience in an outpatient or primary care setting preferred
• Knowledge of nursing assessment, medication administration, injections, immunizations, and other clinical procedures within the LPN scope of practice
• Strong communication, organization, and patient education skills
• Ability to recognize and communicate changes in a patient's condition and respond appropriately
• Ability to use an Electronic Health Record (EHR) system
• Ability to work collaboratively as part of a healthcare team
• Bilingual in English and Spanish is a plus
