Responsibilities

  • Provide patient care under supervision, ensuring safety and comfort for residents.

  • Assist with medications and routine treatments per physician orders and policies.

  • Monitor patient status and escalate concerns to supervising nurse promptly.

  • Ensure strict adherence to safety, infection control, and quality standards.

  • Document care plans, progress notes, and vitals in electronic health records.

  • Collaborate with interdisciplinary team to support patient-centered goals.

  • Maintain high productivity with measurable results and meet performance targets.

  • Provide compassionate, patient-centered care while respecting privacy and dignity.

  • Support continuous learning through training, certifications, and on-the-job development.

  • Assist with patient admissions, discharges, and routine transportation within facility.

  • Maintain equipment readiness and report malfunctions promptly.


Experience/Qualifications

  • Licensed Practical Nurse (LPN) license valid in the applicable state.

  • Experience delivering direct patient care in long-term or facility settings.

  • Proficiency with electronic health records and accurate documentation.

  • Ability to follow physician orders, administer medications, and monitor treatments.

  • Strong infection control, safety, and patient privacy practices.

  • Excellent communication and collaboration with interdisciplinary teams.

  • Commitment to high productivity, meeting performance targets, and measurable outcomes.

  • Compassionate, patient-centered care with respectful handling of dignity and privacy.

  • Willingness to participate in ongoing training, certifications, and professional development.

  • Ability to assist with admissions, discharges, and routine facility transport.
