About the Role:
We Care For You Senior Services is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Southbury, CT. Responsibilities:
  • Provide direct patient care to senior residents in accordance with established care plans and clinical protocols
  • Administer medications and treatments as prescribed, ensuring accurate documentation
  • Monitor and record patient vital signs, symptoms, and changes in condition
  • Collaborate with RNs, physicians, and interdisciplinary care teams to coordinate resident care
  • Assist with wound care, catheter care, and other skilled nursing procedures
  • Communicate effectively with residents, families, and caregivers regarding care plans and health updates
  • Maintain accurate and timely medical records in compliance with state and federal regulations
Requirements:
  • Active LPN license in the state of Connecticut (required)
  • Previous experience in geriatric, long-term care, or senior services settings preferred
  • Strong knowledge of medication administration and clinical nursing procedures
  • Excellent communication, organizational, and critical thinking skills
  • Compassionate, patient-centered approach to elder care
  • Ability to work independently as well as collaboratively within a care team
  • CPR/BLS certification preferred
