Key Responsibilities
- Clinical Duties:
- Perform and document ordered wound care in clinic settings.
- Administer and manage medications as directed by PCP.
- Process specialty pharmacy medication orders and address participant medication issues.
- Conduct medication reviews and manage inventory in clinic.
- Care Coordination:
- Document pre and post-operative clinical touchpoints.
- Conduct high-risk monitoring touchpoints for participants.
- Collaborate with RN Case Manager and PCPs on participant care.
- General Duties:
- Work within the scope of authority and maintain confidentiality.
- Complete all required documentation and training timely.
- Communicate regularly with the supervisor and team regarding workload and priorities.
Required Skills & Experience
- Minimum of one year of nursing experience in a clinical setting with a frail or elderly population.
- Excellent clinical, organizational, and communication skills.
- Ability to work independently and prioritize in a fast-paced environment.
- Experience working with diverse backgrounds and cultures.
- Commitment to unlocking the full potential of our most vulnerable seniors.
Schedule
Monday to Friday, 8:00 AM - 4:30 PM. Candidates must have reliable transportation.
