    Description

    JOB SUMMARY:


    Under the direction of the Director of Wellness, the Resident Services Shift Supervisor (LPN) uses his/her specialized knowledge/training to insure the Resident’s care and management needs are adequately and appropriately assessed, and acts as the liaison between Residents, the Residents Legal Representative, their families, administrative staff, and community-based service providers. Responsibilities include the motivation and supervision of the Resident Services Department personnel, oversight (in the absence of the Director of Wellness) of the day to day operation of the Department to insure compliance with company policy and the State Department of Health regulatory requirements.


    The Shift Supervisor (LPN) works closely with the Director of Wellness and the Director of Reflections. As a member of the multi-disciplinary team, the Shift Supervisor (LPN) contributes to the development of the Resident’s Individual Plan of Care, and actively participates in the supervision of that care.  The Shift Supervisor (LPN) provides primary resident supervision and acts as a support to Residents, oversees (in consultation with the Director of Wellness), admissions and discharge processes; assures Residents are oriented to the community and provides social and emotional support, arranges for needed services; and maintains ongoing communications with all personnel involved in the Resident’s care.


    DUTIES AND KEY RESPONSIBILITIES:

    1. Administrative Services:
    2. Responsible for overall supervision of the building in Executive Director (ED) and The Director of Wellness, the Director of Reflections’ absence.
    3. Must be familiar with and able to implement emergency procedures as outlined in the Bristal Emergency and Disaster Plan.
    4. Responds to all requests for information and all other activities as directed by the Executive Director and/or the Director of Wellness;
    5. Must be familiar with and able to trouble shoot and manage the Emergency Response System:
    6. Assure Residents pendants are correctly entered into the system
    7. Assure pull down cords and pendants are correctly re-set
    8. Assure ERS accountabilities are followed as per policy
    9. Responsible for the operation of the Wellness and Reflections offices in the absence of the ED, the Director of Wellness, and the Director of Reflections including:
    10. Maintains on-going infection control surveillance, OSHA standards, universal precautions, and basic sanitation and safety for all Departments
    11. Assures the accuracy of Resident records and documentation in adherence with company policies as well as regulations of the State Department of Health
    12. Assures the medication management system adheres to company policy and State Department of Health regulations.
    13. Follows the policies and procedures related to incident reporting and notifications as well as State Department of Health Regulations
    14. Assures appropriate reporting of incidents and occurrences
    15. Immediately informs the ED and/or the Director of Wellness or all reportable occurrences
    16. Assists at reception desk and in dining room as needed
    17. Performs other duties as assigned by the ED, the Director of Wellness, the Director of Reflections


    1. Resident Supervision:
    2. Act as Resident advocate and assists staff in their continued service to the Resident
    3. Visit with each Resident to assess their concerns
    4. Refer all Resident complaints and/or concerns to the ED for investigation and resolution
    5. Inform the Director of Wellness of such referrals
    6. Provide oversight to the admission process of the Resident to include:
    7. Appropriateness and accuracy of the Resident’s medical evaluation forwards to the Director of Wellness for sign off on same
    8. If requested by the Director of Wellness or ED, conducts an observation of Resident’s functional capacity forwards to the Director of Wellness for sign off
    9. If indicated, assures completion of Behavior Health Evaluation by appropriate licensed health care professional, at least 30 days prior to admission and annually
    10. Provide initial and ongoing orientation to facility for new residents;
    11. Acclimate resident and introduce them to other residents.
    12. Encourage Residents to participate in social activities.
    13. Inform administrative staff of the general wellbeing of Residents as well as their ability to live in the assisted living setting.
    14. Conduct evaluations, as delegated by the Director of Wellness, to determine ongoing status of Residents service needs forwards such evaluations to the Director of Wellness for review and sign off
    15. Communicate change in Resident status to the Director of Wellness
    16. Assure Resident is receiving needed assistance
    17. Collaborate with multidisciplinary team of providers, as well as the Resident and/or Resident’s Legal Representative in the development of the Resident’s Individualized Service Plan
    18. Alert all staff to a Resident’s changing needs
    19. Monitor the Wellness and Reflections Communication Logs at the start and end of his/her shift and provide appropriate follow-up and documentation for any unusual occurrences or changes in resident’s status
    20. Ensure all corporate policies and procedures and State Department of Health Regulations are followed and maintained
    21. Refer Resident Service problems to the appropriate Department Directors, the Director of Case Management and Administration as necessary
    22. Maintain accurate Resident records in compliance with company policy and regulatory rule
    23. Supervise Wellness and Reflections staff and ensures that Residents receive appropriate care as defined in each Resident’s service plan
    24. Responsible for performing and monitoring procedures for Medication Management:
    25. Transcribing Doctors’ orders and accuracy of MARS
    26. Medication supervision, self-administration, medication re-ordering and ensuring medication accountability
    27. Scheduled checks of Residents rooms (supervises).
    28. Responsible for conducting Resident Self Medication reviews as delegated by the Director of Wellness:
    29. Follow process and standard with regard to reviews:
    30. Coordinate Resident care through collaboration with ED, The Director of Wellness, the Director of Reflections, the Director of Case Management, Department Directors, Resident’s physician, Resident and Resident’s Representative, and other third-party providers
    31. Meet regularly with Resident Services Department leadership and Administration to exchange information about the Residents and personal care services.
    32. Exchange information about the status of Residents and personal care services Resident Services staff:
    33. Facilitate stand-up and shift change reports with Wellness and Reflections staff
    34. Assist with hospital transfers and discharges and liaise to hospital social worker
    35. Act as liaison with Resident’s personal physician and other healthcare providers
    36. Performs other duties as assigned by the ED, Director of Case Management, and Resident Services Department


    1. Staff Education, Development, Supervision and Evaluation:
    2. Assist in the orientation, training, mentoring, and evaluation of Resident Services Department staff and privately hired aides and companions:
    3. Assure new hires are receiving the mentoring and training needed to be successful in their roles
    4. Conduct on-the-spot in-service when performance issue identified and document such trainings
    5. As delegated by the Director of Wellness, schedule and provide in-services and trainings to staff
    6. Report all employee and private hire performance issues to the Director of Wellness/ED
    7. Supervise and provide evaluative input to Resident Services Department leadership regarding staff performance
    8. Reports all employee issues to the Director of Wellness and/or Executive Director.
    9. Provide ongoing supervision of the Resident Services Department staff, privately hired aides and companions to assure Residents are receiving appropriate care as defined in the Residents’ service plans:
    10. Responsive to individual and global staffing needs and be willing to “step – in” when scheduling demands require:
    11. Assist Residents with their medications as needed
    12. Assist Residents with care as needed
    13. Assure privately hired aides are providing needed care as described in the Residents’ service plan and that care is provided in a person-centered approach
    14. Assure privately hired companions are not assisting Residents with Activities of Daily Living beyond escorting, reminders for meals and activities, and recreational activities
    15. Conducts appropriate reviews and address / report findings:
    16. ERS response time reports
    17. Task scanning report at mid and end of shift
    18. Walks the community to assure staff are providing care, identification of maintenance and/or housekeeping issues, and address and/or report findings
    19. Be a presence in the dining room during meals to assure appropriate hygiene of Residents and identification of any Resident / staff issues
    20. Conducts periodic health seminars for the Residents and/or Residents’ families in collaboration and coordination with the Lifestyle Director.


    1. Annual Training Requirements
    2. A minimum of 12 hours of mandatory in-service
    3. Maintain CPR, AED, First Aide certifications
    4. Maintain CEUs as needed to maintain LPN licensure



    QUALIFICATIONS:

    1. Graduate from an accredited school of nursing (LPN)
    2. Certification in Dementia Care (preferred), CPR and AED (required in NJ), First Aide (required in NY)
    3. Minimum of 3 – 5 years supervisory experience in a geriatric setting
    4. Experience supervising staff
    5. Use of compassion, tact and courtesy relating with Residents, Resident families, employees and visitors
    6. Must be emotionally, mentally and physically able, with or without reasonable accommodations, to provide required services to residents, including verbally communicating with residents.
    7. Must be able to speak, read, and write in English, which is the predominant language of a majority of our residents.
    8. Must be able to listen attentively to residents' requests and preferences, ensuring exceptional customer service.
