At Woodbury Heights Nursing and Rehabilitation center we are looking to build the best team to provide optimum care to our LTC and Rehabilation residents .

We take pride in our compassionate, patient-centered approach, and our team of dedicated LPNs is at the heart of everything we do.

Schedules:

• Every weekend or every other weekend

• 7am to 3pm, 3pm to 11pm or 11pm to 7am

•  shifts on weekdays available as well. 

Responsibilities, but not limited to:

• Provide compassionate, patient-centered care to residents, helping them live comfortably and with dignity.

• Administer medications and treatments as prescribed by physicians.

• Monitor and document vital signs, symptoms, and changes in resident conditions.

• Assist in developing and implementing personalized care plans.

• Collaborate with RNs, physicians, and other healthcare professionals to ensure the highest standard of care.

• Respond to medical emergencies and provide immediate support.

• Maintain accurate and up-to-date medical records.

• Promote a positive, supportive environment for both residents and team members.

Requirement:

• Valid LPN license in New York State

• Current CPR and First Aid certification

• 1 year of experience in long term care.
