As a Territory LPN, you'll work independently while being supported by an experienced clinical team. You'll build meaningful relationships with patients and families, make a direct impact on patient outcomes, and enjoy variety in your day-to-day work.

Position Details

  • Monday–Friday schedule

  • Average of 6 patient visits per day

  • Territory Nurse role covering Stark County and portions of Tuscarawas and Carroll Counties

  • Daily visits are scheduled geographically to maximize efficiency whenever possible

  • Participation in agency on-call and weekend rotation required

  • Two holidays per year required
     

What You'll Do

  • Provide skilled nursing care in patients' homes under physician orders and RN direction

  • Assess and monitor patient conditions

  • Administer medications and treatments

  • Educate patients and families on disease management and preventative care

  • Collaborate with physicians, case managers, and interdisciplinary team members

  • Complete timely clinical documentation in accordance with agency and Medicare guidelines

  • Help reduce hospital readmissions through patient education and proactive care
     

Qualifications

  • Current Licensed Practical Nurse (LPN) license

  • Minimum one year of acute care experience required

  • Home health experience preferred

  • Strong assessment, communication, and critical thinking skills

  • Valid driver's license and active auto insurance

  • Ability to travel throughout the assigned territory
