    Job Summary:
    Tryfacta is seeking a LPN for our client in Florence, WI 54121. This is a 6 weeks assignment. If you meet the qualifications listed below and are interested, please Apply Now!

    Position Title: LPN
    Location:Florence, WI 54121
    Duration: 6 weeks
    Work Schedule: Nights | 10:00 PM – 6:30 AM | 8-Hour Shifts

    Responsibilities for this position include, but are not limited to:

    • Provide direct nursing care to residents in a long-term care/skilled nursing setting during the night shift.
    • Administer medications, treatments, and therapies as prescribed while monitoring resident responses.
    • Perform resident assessments, monitor vital signs, and promptly report changes in condition to the RN or provider.
    • Assist with activities of daily living (ADLs), mobility, transfers, and resident comfort while promoting dignity and independence.
    • Maintain accurate documentation of nursing care, medications, and resident status in PointClickCare (PCC) .
    • Collaborate with physicians, RNs, CNAs, therapists, and other interdisciplinary team members to provide quality resident care.
    • Ensure compliance with infection prevention, resident safety, and facility policies and procedures.
    • Respond appropriately to emergencies and provide compassionate, resident-centered care throughout the shift.
    • Active Wisconsin LPN License or Compact LPN License (if applicable).
    To be considered for this position, you should have: [Skills, Education, or Experience]
    • Minimum 1 year of recent Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred.
    • Current BLS Certification preferred.
    • Experience with PointClickCare (PCC) or similar electronic medical record (EMR) system preferred.
    • Strong skills in medication administration, patient assessment, wound care, and documentation.
    • Excellent communication, teamwork, and time management skills.
    • Ability to work Night Shift (10:00 PM – 6:30 AM) .
    • 48 guaranteed hours per week with overtime available at the facility's discretion .
