Responsibilities for this position include, but are not limited to:
- Provide direct nursing care to residents in a long-term care/skilled nursing setting during the night shift.
- Administer medications, treatments, and therapies as prescribed while monitoring resident responses.
- Perform resident assessments, monitor vital signs, and promptly report changes in condition to the RN or provider.
- Assist with activities of daily living (ADLs), mobility, transfers, and resident comfort while promoting dignity and independence.
- Maintain accurate documentation of nursing care, medications, and resident status in PointClickCare (PCC) .
- Collaborate with physicians, RNs, CNAs, therapists, and other interdisciplinary team members to provide quality resident care.
- Ensure compliance with infection prevention, resident safety, and facility policies and procedures.
- Respond appropriately to emergencies and provide compassionate, resident-centered care throughout the shift.
- Active Wisconsin LPN License or Compact LPN License (if applicable).
To be considered for this position, you should have: [Skills, Education, or Experience]- Minimum 1 year of recent Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred.
- Current BLS Certification preferred.
- Experience with PointClickCare (PCC) or similar electronic medical record (EMR) system preferred.
- Strong skills in medication administration, patient assessment, wound care, and documentation.
- Excellent communication, teamwork, and time management skills.
- Ability to work Night Shift (10:00 PM – 6:30 AM) .
- 48 guaranteed hours per week with overtime available at the facility's discretion .
