Rate: $30-$33/hour
Sign-on bonus: Up to $3,000
Shift: 11PM-7AM, Weekdays, Full-Time ($3 Shift Differential)

North Gate Health Care Facility is seeking an experienced LPN Unit Coordinator to support daily clinical operations and provide leadership on the unit. This role is ideal for an LPN who enjoys coordinating care, supporting staff, and ensuring high-quality resident outcomes.

Key Responsibilities:

• Coordinate unit workflows and resident care
• Support nursing staff and ensure regulatory compliance
• Communicate with interdisciplinary teams and leadership
• Assist with documentation and clinical follow-up

Qualifications:

• Active LPN license
• Prior SNF/LTC or charge experience preferred
• Strong organization and communication skills
