Licensed Practical Nurse (LPN) Visit Nurse – Lima, OH

$26 - 30 per visit | Flexible Scheduling | Part-Time

As a Licensed Practical Nurse (LPN) Visit Nurse with Interim HealthCare Home Health, you'll provide one-on-one care to patients in the comfort of their own homes. This is a great opportunity for an LPN who enjoys building relationships with patients, working independently, and having flexibility in their schedule.

Rather than being tied to a facility or one patient for an entire shift, our Visit Nurses travel to patients' homes to complete skilled nursing visits and provide the care they need.

As a Licensed Practical Nurse (LPN) Visit Nurse, You Will:

• Provide in-home direct patient care according to the provider's prescribed plan of care and nursing scope of practice.

• Complete skilled nursing visits and assessments in the patient's home.

• Monitor patient progress and communicate changes in condition to the appropriate providers.

• Implement and follow comprehensive care plans based on individual patient needs.

• Provide patient and family education related to the patient's care.

• Complete accurate clinical documentation in accordance with agency policies and Medicare/Federal guidelines.

• Collaborate with the care team to promote continuity of care and help reduce preventable hospital readmissions.

• Travel to patient homes throughout the Lima service area.

To Qualify for This Position, You Will Need:

• Graduate of an approved practical nursing program.

• Current, unrestricted LPN license in the state of Ohio.

• Current CPR/AED/BLS/First Aid certification.

• Reliable transportation to and from patient homes.

• At least one years of LPN experience in a home health or similar setting preferred OR one year of nursing and wound care experience.

• Strong communication, organization, and clinical assessment skills.

• Ability to work independently while following the patient's plan of care.
