LPN – Full Time (Days/Evenings Rotation) [Contract]
Location: New York 13165
Pay: $1,396 – $1,474
Schedule: Full Time (5x8s), Days/Evenings as needed, EOW and holidays required
Orientation: Must start on a Tuesday/Thursday for full-shift orientation
Active New York LPN License
BLS Certification (AHA or RQI)
Minimum 1 year LPN experience (LTC preferred)
Experience with geriatric patient population
Ability to work rotating schedule including weekends and holidays
Provides direct and indirect patient care under RN supervision
Performs ongoing assessments and communicates changes to RN/team
Assists with care planning, implementation, and evaluation
Collaborates with interdisciplinary team to support patient outcomes
Maintains competency through ongoing training and education
Delivers care to adult and geriatric residents in a safe, compliant manner
May have exposure to bloodborne pathogens; follows all safety protocols
