Licensed Practical Nurse (LPN) – Full Time Evenings (Contract)
Location: California 95240
Pay: $2,594 – $2,905/week
Full Time Evenings: 5x8-hour shifts/week
Every other weekend (EOW) and holidays required
Must attend full-shift orientation on Tuesday or Thursday
Rotations: Day / Evening / Night / Weekend
Provide direct and indirect skilled nursing care under RN supervision for adult and geriatric residents
Collect resident data and communicate findings to RN and interdisciplinary team
Assist in developing and updating age-appropriate, culturally relevant care plans
Collaborate with residents, families, and care team to ensure quality outcomes
Maintain competency through continuing education and participate in staff mentoring/orientation
Follow safety protocols; potential exposure to blood or body fluids
NY LPN Licensure – Required
BLS certification (AHA or RQI) – Required
Minimum 1-year LPN experience in a long-term care setting – Required
