Responsibilities

  • Provide hands-on nursing care to patients as an LPN, per care plans.

  • Document patient status and update charts with accuracy and timeliness.

  • Administer medications and treatments under physician orders and policy.

  • Collaborate with healthcare team to coordinate patient care and outcomes.

  • Monitor patient responses and escalate concerns per protocol.

  • Adhere to regulatory and safety standards, including infection control.

  • Proactively identify opportunities for professional development and skill advancement.

  • Meet performance targets and contribute to pipeline-driven goals for the area.

  • Educate patients and families on care plans, treatments, and follow-up steps.

  • Maintain accurate mileage, time, and activity logs for performance visibility.

  • Participate in on-site team meetings to review progress and share insights.


Experience/Qualifications

  • Active LPN license in the relevant state with current CPR certification.

  • Experience providing hands-on nursing care per care plans and procedures.

  • Proficiency documenting patient status and updating electronic health records accurately.

  • Ability to administer medications and treatments under physician orders and policy.

  • Strong collaboration skills with healthcare team to coordinate patient care.

  • Observation skills to monitor patient responses and escalate per protocol.
