Responsibilities
Provide hands-on nursing care to patients as an LPN, per care plans.
Document patient status and update charts with accuracy and timeliness.
Administer medications and treatments under physician orders and policy.
Collaborate with healthcare team to coordinate patient care and outcomes.
Monitor patient responses and escalate concerns per protocol.
Adhere to regulatory and safety standards, including infection control.
Proactively identify opportunities for professional development and skill advancement.
Meet performance targets and contribute to pipeline-driven goals for the area.
Educate patients and families on care plans, treatments, and follow-up steps.
Maintain accurate mileage, time, and activity logs for performance visibility.
Participate in on-site team meetings to review progress and share insights.
Experience/Qualifications
Active LPN license in the relevant state with current CPR certification.
Experience providing hands-on nursing care per care plans and procedures.
Proficiency documenting patient status and updating electronic health records accurately.
Ability to administer medications and treatments under physician orders and policy.
Strong collaboration skills with healthcare team to coordinate patient care.
Observation skills to monitor patient responses and escalate per protocol.
