Position Overview
- Shift: PM Shift (including 72-hour PM every other weekend)
- Schedule: 3240 hours per week

Key Responsibilities
- Provide direct patient care to residents in a skilled nursing facility
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes to the care team
- Assist with daily living activities, including hygiene and mobility
- Maintain accurate and timely documentation
- Collaborate with RNs, CNAs, and healthcare staff to ensure quality care
- Support patient comfort, safety, and dignity at all times
Education & Certification Requirements
- Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license
- Graduate of an accredited practical nursing program
- Current BLS (Basic Life Support) certification
- Minimum of 1 year of experience in skilled nursing or long-term care preferred
- Strong clinical, communication, and organizational skills
