We are seeking a compassionate and skilled LTC Outpatient Therapist to join our team. The ideal candidate will provide high-quality therapeutic services to long-term care (LTC) patients in an outpatient setting, helping them improve their physical, emotional, and mental well-being.
Responsibilities
Conduct comprehensive assessments of patients' physical, emotional, and cognitive needs.
Develop individualized treatment plans based on patient assessments and goals.
Provide evidence-based therapeutic interventions to LTC patients in an outpatient setting.
Monitor patient progress and adjust treatment plans as necessary.
Collaborate with interdisciplinary teams including physicians, nurses, social workers, and family members to ensure coordinated care.
Document patient sessions, progress notes, and treatment plans accurately and timely in compliance with regulatory standards.
Educate patients and their families about treatment goals, techniques, and strategies for managing conditions.
Maintain patient confidentiality and adhere to ethical and professional standards.
Participate in continuing education and professional development activities.
Qualifications
Master's degree in Physical Therapy, Occupational Therapy, Speech Therapy, Psychology, Social Work, or related field.
Valid and current licensure or certification as required by state regulations.
Experience working with long-term care patients in an outpatient or similar setting preferred.
Strong knowledge of therapeutic techniques and interventions relevant to LTC populations.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Proficient in electronic health records (EHR) and documentation standards.
Compassionate, patient-centered approach to care.
Work Environment
This position primarily involves outpatient clinical work with LTC patients. The therapist will work in a healthcare facility equipped to support therapeutic services and patient care.
