A-Line Staffing is now hiring a LTSS Field Case Manager in the Jacksonville/Wilmington, North Carolina areas. The LTSS Field Case Manager would be working for a Major Fortune 500 Company and has career growth potential.
??? Schedule: Monday???Friday, 8:00 AM ??? 5:00 PM EST
??? Pay Range: $36-42 (Hourly), plus mileage reimbursement (71 cents per mile)
??? Remote with Field Work:
??? Manage a caseload of members requiring Long-Term Services and Supports (LTSS)
??? Conduct monthly and quarterly member outreach (in-person and telephonic)
??? Perform comprehensive member assessments and document care plans
??? Coordinate with caregivers, providers, and interdisciplinary teams
??? Monitor service delivery to ensure no gaps in care
??? Authorize and coordinate referrals for services
??? Identify deficiencies in care plans and implement corrective actions
??? Assist with discharge planning and coordination of acute and long-term care
??? Support members with complaints and appeals resolution
??? Collaborate with physicians, discharge planners, and community resources
Required:
??? Bachelor???s Degree
??? Active RN and/or LCSW/LCSW-A/BSN/ADN
??? 2???4+ years of physical health care management experience (hospital CM, utilization management, home health)
??? Valid driver???s license
??? Strong case management and care planning experience
Strong technical skills (Microsoft Office, EMR systems)
Bedside care management experience
Preferred:
??? 4???6+ years of experience in healthcare management
??? Background in hospital case management, home health, or discharge planning
??? Experience in community health/member-facing roles
??? Familiarity with electronic medical records (EMR) systems
??? Proficiency in Microsoft Office
Disqualifiers:
??? Psych-only experience (e.g., counseling without physical health background)
??? New graduates without hands-on experience
??? Experience limited to nursing homes or rehab facilities
