Develop or assist with developing personalized service care plans/service plans for long-term care members.
Educate members and their families/caregivers on services and benefits available to meet member needs.
Requirements:
Bachelor’s degree in social work, Health Services, Behavioral Science, or related field, or equivalent experience (minimum 2 years) in a managed care environment or working with people who have disabilities and/or with vulnerable populations who have chronic or complex conditions.
LPN, LVN, or LCSW preferred.
Proficiency in Microsoft Outlook, Microsoft Teams, Zoom, Microsoft Word, Microsoft Excel, SharePoint, and Internet.
Preferred Skills:
2 years of experience in a managed care environment or working with people who have disabilities and/or with vulnerable populations who have chronic or complex conditions.
