LTSS Service Care Manager

Summary:

• Work Mode: Remote and Field Visits are required throughout New Castle, mileage will be reimbursed by the company.
• Duration: 6 months, possible extension or conversion.
• Schedule is planned from Monday-Friday 8am – 5pm | No OT expectations

Responsibilities:

• Complete level of care redetermination.
• Conduct a mixture of in-home care visits.
• Support other case managers.
• Assist the case management team due to high increase in membership.

Requirements:

• Bachelor's degree and 2 – 4 years of related experience.
• Delaware/Compact RN License.
• Driver’s license.
• Must have a home workspace.
• Experience with Level of Care Redetermination, Transition of Care, and Case Management.
• Computer literate.
• Experience in evaluating older adults/disabled level of functioning.
• Experience with medication reconciliation.

Disqualifiers:

• Location should be within Delaware.

Performance Indicators:

• 4-5 assessments per week for redeterminations.
• 4-5 daily assessments.

Preferred Skills:

• Service Care Manager experience.
