Summary:
- Work Mode: Remote and face-to-face visits to members in Texas (Dallas, Nueces, and Central SDAs areas)

Responsibilities:
- Assist in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.
- Develop or assist with developing personalized service care plans/service plans for long-term care members.
- Educate members and their families/caregivers on services and benefits available to meet member needs.
- Manage member cases through outreach, face-to-face visits, and care coordination.
- Complete assessments, coordinate services, and ensure members receive needed care while maintaining documentation and compliance standards.
Requirements:
- Education/Certification: Required: LVN or LBSW
- Licensure: Required: LVN or LBSW
- Minimum 2 years of experience required
- Top must-have hard skill: Time management
