LTSS Service Care Manager

Summary:

• Work Mode: Remote and face-to-face visits to members in Texas (Dallas, Nueces, and Central SDAs areas)
• Duration: 12 months
• Shift: Mon - Fri 8am - 5 PM CST
• Target start date: Aug 31, 2026

Responsibilities:

• Assist in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.
• Develop or assist with developing personalized service care plans/service plans for long-term care members.
• Educate members and their families/caregivers on services and benefits available to meet member needs.
• Manage member cases through outreach, face-to-face visits, and care coordination.
• Complete assessments, coordinate services, and ensure members receive needed care while maintaining documentation and compliance standards.

Requirements:

• Education/Certification: Required: LVN or LBSW
• Licensure: Required: LVN or LBSW
• Minimum 2 years of experience required
• Top must-have hard skill: Time management

Preferred Skills:

• Prior SC/CM experience
