Summary:
Location: Region 5 (Bladen, Brunswick, Columbus, Cumberland, Harnett, Hoke, Lee, Montgomery, Moore, New Hanover, Pender, Richmond, Robeson, Sampson, Scotland counties)
Work Mode: Remote
Duration: 6 months, possibility of conversion but not guaranteed
Monday-Friday 8-5, Eligible for OT
Responsibilities:
Develop, assess, and coordinate holistic care management activities to enable quality, cost-effective healthcare outcomes.
Complete face-to-face and telephonic assessments with members to determine ADL assistance needs.
Coordinate care between members and care partners to address medical care needs.
Develop personalized service care plans for long-term care members.
Educate members and their families/caregivers on services and benefits available to meet member needs.
Requirements:
Bachelor's degree required.
NC RN licensure required.
Valid driver’s license and personal vehicle required.
Comfortable traveling within a 2-hour radius to members’ homes on a weekly basis.
Experience in assisting people in navigating the health system.
Preferred Skills:
CCM certification.
Strong interpersonal skills and ability to build positive relationships with team members, leaders, and members.
Excellent communication, problem-solving, and physical assessment skills.
Demonstrate empathy, patience, and respect for diverse perspectives.
