Overview:

Lung Transplant Advanced Practice Provider (NP/PA) - Piedmont Atlanta

 

The Lung Transplant Advanced Practice Provider (APP) is responsible for the comprehensive management of patients undergoing lung transplantation across the continuum of care, including pre-transplant evaluation, inpatient hospitalization, peri-operative management, and long-term outpatient follow-up.

 

The APP functions as an integral member of a multidisciplinary transplant team that may include transplant pulmonologists, cardiothoracic surgeons, pharmacists, social workers, transplant coordinators, respiratory therapists, and nursing staff.

Responsibilities:

Essential Duties and Responsibilities

 

Clinical Care:

  • Perform comprehensive patient assessments, including histories and physical examinations
  • Manage pre- and post-lung transplant patients in both inpatient and outpatient settings
  • Conduct daily rounds on transplant patients
  • Evaluate transplant candidacy and assist in pre-transplant workups
  • Monitor immunosuppressive therapy and manage complications
  • Order, interpret, and evaluate laboratory studies, imaging, pulmonary function tests, and other diagnostic procedures
  • Develop differential diagnoses and treatment plans
  • Prescribe medications and therapeutic interventions in accordance with state regulations and institutional protocols
  • Assist with discharge planning and continuity of care

Care Coordination:

  • Collaborate with attending physicians and multidisciplinary transplant team members
  • Coordinate care transitions between ICU, step-down, clinic, and home settings
  • Participate in transplant selection committee meetings and multidisciplinary conferences
  • Facilitate communication with referring physicians and outside hospitals

Patient & Family Education:

  • Educate patients and caregivers regarding:
    • Lung transplant procedures
    • Medication regimens
    • Infection prevention
    • Lifestyle modifications
    • Post-transplant expectations
  • Provide ongoing counseling regarding adherence and chronic disease management

Documentation & Compliance:

  • Maintain timely and accurate electronic medical records
  • Complete transplant evaluation summaries and discharge documentation
  • Ensure compliance with UNOS, CMS, and institutional transplant regulations

Quality & Research:

  • Participate in quality improvement initiatives
  • Support transplant program accreditation activities
  • Participate in clinical research, education, and protocol development when applicable

Work Schedule:

  • Full-time
  • Mix of inpatient and outpatient responsibilities
  • Rotating weekends and holidays required
  • Call coverage required as program structure expands
Qualifications:

Required Qualifications:

  • Master’s or Doctoral degree from accredited NP or PA program
  • Current state licensure as:
    • Nurse Practitioner (NP), or
    • Physician Assistant (PA-C)
  • DEA registration eligibility
  • BLS and ACLS certification
  • Ability to work in high-acuity clinical environments

Preferred Qualifications:

  • Experience in:
    • Pulmonary medicine
    • Critical care
    • Cardiothoracic surgery
    • Solid organ transplantation
  • Acute Care NP certification preferred for inpatient roles
  • Prior transplant experience strongly preferred
