JOB SUMMARY
The primary function of the LVN in the call center is to perform prescription refill processing under approved Shasta Community Health Center (SCHC) protocols while providing telephone-based clinical support within the LVN scope of practice.
JOB DUTIES AND RESPONSIBILITIES
Prescription Refill Management
· Process and authorize medication refill requests in accordance with the SCHC Medication Refill Standard Operating Procedure and approved clinical protocols.
· Review patient records to verify medication, visit, laboratory, diagnosis, and other applicable refill requirements.
· Process refills within established timeframes.
· Identify and escalate refill requests that do not meet protocol requirements or require provider review.
· Coordinate required laboratory testing, appointments, and follow-up related to medication refills.
· Communicate with patients, pharmacies, and clinical staff regarding refill-related needs.
· Document refill activity accurately in the EHR.
· Complete required medication refill training and competency requirements.
Call Center Nursing
· Receive and respond to incoming patient calls with the LVN scope of practice.
· Provide information, education, and assistance for routine clinical and medication related questions.
· Address patient needs using approved protocols, standing orders, and established workflow.
· Identify calls that require nursing assessment, clinical triage, or higher-level clinical decision-making.
· Escalate calls requiring clinical triage or nursing assessment to the department RN, provider, or TeamHealth/Access Nurse, as appropriate.
· Coordinate appointments, laboratory testing, medication follow-up, and other patient needs identified during calls.
· Document patient calls, interventions, education and follow-up in EHR.
· Communicate with team members to ensure appropriate follow-up and continuity of care.
· Manage assigned task boxes and assist with task management and clinical support needs for nurses across department, within LVN scope.
· All other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
· Knowledge of nursing process, scope of practice and standards of care.
· Ability to pass a written medication quiz.
· Passes Medication Refill exam.
· Basic computer proficiency in Windows environment.
· Typing 35 WPM or greater.
EDUCATION & EXPERIENCE
· California LVN license upon hire and maintain current.
· Current CPR/BLS certification upon hire and maintain current.
PREFERRED
Minimum of one year's experience in acute care nursing.
· Previous experience working in an outpatient clinic or doctor's office.
· Typing 45 WPM or greater.
