Job Overview
We are seeking an LVN to join our team! You will be responsible for delivering high quality care to a patient in Long Beach, CA. The patient needs hourly shift care at their home. Knowledge of quadriplegic patients is preferred. Patience, kindness and a genuine attitude of compassionate care are a MUST. The shift opening is every Monday night from 11PM-7AM. Spanish-speaking is a PLUS.


Responsibilities


  • Follow the plan of care set by the RN Case Manager
  • Administer medications and treatments as prescribed, including tube feeding and acute pain management interventions.
  • Provide skilled nursing care in various settings, including home health, ensuring a safe and supportive environment for patients.
  • Collaborate with physicians, social workers, and other healthcare professionals to coordinate comprehensive care for patients and their families.
  • Educate patients and families about disease processes, treatment options, and end-of-life care decisions.
  • Monitor patient conditions closely for any changes or complications related to infectious disease care or other health issues.
  • Document all patient interactions accurately in compliance with regulatory standards.
Qualifications


  • Current LVN nursing license in the state of practice.
  • Experience in primary care, urgent care, emergency room (ER), or laboratory settings is preferred.
  • Proficiency in medical imaging techniques is a plus.
  • Strong understanding of hospice philosophy and palliative care principles.
  • Excellent communication skills to effectively interact with patients, families, and healthcare team members.
  • Ability to handle emotional situations with sensitivity and professionalism.
  • Must be able to work flexible hours as needed to meet patient needs.
