Experience/Qualifications

  • Licensed Vocational Nurse (LVN) certification in good standing.

  • 2 years Clinic Experience

  • Proven experience in care coordination within a healthcare setting.

  • Commitment to continuous professional development and learning.

  • Strong communication skills to effectively collaborate with healthcare teams.

  • Ability to analyze patient data and adjust care plans accordingly.

  • Ability to work independently while being accountable for results.

  • Knowledge of healthcare regulations and compliance standards.

  • Proficiency in electronic health record (EHR) systems.

  • Strong organizational skills to manage multiple patient care plans.

  • Capability to educate patients and families on health management.


Responsibilities

  • Coordinate patient care plans and ensure effective communication among healthcare providers.

  • Monitor patient progress and adjust care plans as necessary to achieve health goals.

  • Maintain accurate and up-to-date patient records in compliance with healthcare regulations.

  • Collaborate with multidisciplinary teams to optimize patient care and outcomes.

  • Ensure compliance with all healthcare policies, procedures, and standards.

  • Educate patients and families on care plans and health management strategies.

  • Identify and address potential barriers to patient care and propose solutions.

  • Develop and maintain strong relationships with patients and their families.

  • Participate in continuous professional development to enhance skills and knowledge.

  • Utilize data and metrics to assess care effectiveness and improve service delivery.

  • Report on care outcomes and provide feedback to improve patient services.

  • Contribute to the development of care protocols and best practices.
