Overview:
We are seeking compassionate and skilled LVN/LPNs who are ready to make a meaningful impact in a structured, team-driven care environment. This is a high-impact role where your clinical expertise, attention to detail, and patient-centered approach will directly contribute to the health, safety, and well-being of individuals in care.
What you will be doing:
As an LVN/LPN you will provide hands-on nursing care while working collaboratively with an interdisciplinary team. Key responsibilities include:
Administer medications and treatments via oral, rectal, injectable, and other prescribed routes
Perform technical nursing procedures in accordance with facility protocols and state regulations
Accurately document patient care, observations, and communications in the medical record
Communicate effectively with team members to ensure continuity and quality of care
Respond promptly to emergency situations, ensuring patient safety at all times
Monitor patients during treatments and procedures, reporting any changes in condition
Collect lab specimens and perform approved screening tests (upon competency validation)
Educate patients on health goals and care plans
Assist with repositioning patients to support treatment delivery and comfort
Maintain adequate supplies and ensure readiness for patient care
Manage patient records within Electronic Health Records (EHR) or paper charts
Maintain consistent attendance and flexibility to support rotating schedules, extended shifts, or overtime as needed
What you must have:
Active LVN/LPN license in Texas (or compact state eligibility)
Strong communication skills, both written and verbal
Proficiency with computers and Electronic Health Records preferred
Experience in IDD and/or psychiatric care is highly preferred
Ability to work independently while contributing to a collaborative team environment

Assignment Details
Open Roles: 2 LVN/LPN positions
Schedule: 40 hours per week | No overtime
Shift Options:
7:00 AM – 7:00 PM (with one 4-hour shift: 7:00 AM – 11:00 AM)
OR 4:00 PM – 12:00 AM (5 days per week)
Schedule Type: Rotating
Contract Length: Minimum 6 months (flexibility for returning nurses)
