    Clinical Care Coordinator (LVN)


    Field-Based Position | San Fernando Valley

    $35–$40/hr + Mileage Reimbursement

    Position Overview


    The Clinical Care Coordinator (LVN) serves as a liaison between patients, families, referral sources, and the hospice/palliative care team. This field-based role provides education, conducts nursing assessments, coordinates admissions, and helps ensure patients receive timely, compassionate care.


    Responsibilities


    • Conduct nursing assessments and admissions
    • Review consents along with other documents and assists with election of hospice benefit.
    • Educate patients and families about hospice and palliative care
    • Coordinate care transitions from hospitals and facilities
    • Collaborate with interdisciplinary teams
    • Complete documentation using EMR systems


    Qualifications


    • Current California LVN License
    • 1+ Year Nursing Experience, hospice experience highly preferred
    • Strong Communication Skills
    • Reliable Transportation & Valid Driver’s License
    • Bilingual Spanish Preferred
