ComForCare Home Health Care (North San Diego) is looking for a compassionate and dedicated LVN Field Nurse to join our growing team in San Diego, CA. This is a rewarding opportunity to make a real difference in patients lives by delivering high-quality, personalized care right in the comfort of their homes. Responsibilities:

• Provide skilled nursing care to home health patients in accordance with physician-approved care plans
• Conduct patient assessments, monitor vital signs, and document clinical findings accurately
• Administer medications, wound care, IV therapy, and other treatments as ordered
• Educate patients and family members on disease management, medications, and home safety
• Collaborate with physicians, RNs, and interdisciplinary care team members to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with state and agency standards
• Travel to patient homes throughout the North San Diego service area

Requirements:

• Active and unrestricted LVN license in the state of California
• Minimum 1 year of clinical nursing experience; home health experience preferred
• Current CPR/BLS certification
• Valid California drivers license, reliable transportation, and active auto insurance
• Strong clinical assessment, wound care, and medication administration skills
• Excellent communication, time management, and organizational abilities
• Compassionate, self-motivated, and comfortable working independently in the field

Compensation: $39.00 - $41.00 per hour
