About the Role:
ComForCare Home Health Care (North San Diego) is looking for a compassionate and dedicated LVN Field Nurse to join our growing team in San Diego, CA. Responsibilities:
- Provide skilled nursing care to home health patients in accordance with physician-approved care plans
- Conduct patient assessments, monitor vital signs, and document clinical findings accurately
- Administer medications, wound care, IV therapy, and other treatments as ordered
- Educate patients and family members on disease management, medications, and home safety
- Collaborate with physicians, RNs, and interdisciplinary care team members to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with state and agency standards
- Travel to patient homes throughout the North San Diego service area
Requirements:
- Active and unrestricted LVN license in the state of California
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Current CPR/BLS certification
- Valid California driver's license, reliable transportation, and active auto insurance
- Strong clinical assessment, wound care, and medication administration skills
- Excellent communication, time management, and organizational abilities
- Compassionate, self-motivated, and comfortable working independently in the field
