About the Role: ProRehab HealthCare Group of Companies is looking for a compassionate and skilled Licensed Vocational Nurse (LVN) to join our dedicated care team in Pleasanton, CA. Responsibilities:
Provide direct patient care including administering medications, wound care, and treatments as directed by physicians
Monitor and document patient vital signs, health status, and progress in medical records
Collaborate with RNs, physicians, and interdisciplinary teams to develop and implement patient care plans
Assist with patient assessments and report changes in condition to supervising clinical staff
Educate patients and families on care procedures, medications, and discharge instructions
Maintain accurate and timely clinical documentation in compliance with regulatory standards
Ensure a safe, clean, and therapeutic environment for patients and staff
Requirements:
Active and unrestricted LVN license in the state of California
Current CPR/BLS certification
Prior clinical experience in a healthcare, rehab, or long-term care setting preferred
Strong knowledge of nursing procedures, medication administration, and infection control
Excellent communication, critical thinking, and patient advocacy skills
Ability to work collaboratively in a fast-paced, team-oriented environment
Proficiency with electronic health records (EHR) systems
