LVN/LPN - LTC

Job Summary

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care in a Long-Term Care and Skilled Nursing Facility setting. The LPN will work closely with residents, physicians, registered nurses, and interdisciplinary teams to ensure safe, effective, and patient-centred care while maintaining compliance with facility policies and state regulations.

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor, assess, and document residents' health status and changes in condition.
• Perform routine nursing procedures, including wound care, dressing changes, and infection control measures.
• Assist residents with activities of daily living (ADLs) and promote resident comfort and safety.
• Maintain accurate and timely documentation in accordance with facility standards.
• Collaborate with RNs, physicians, CNAs, and other healthcare professionals to develop and implement care plans.
• Respond appropriately to resident emergencies and changes in condition.
• Educate residents and family members regarding treatment plans and healthcare needs.
• Ensure compliance with federal, state, and facility regulations.
• Supervise and support nursing assistants as needed.

Required Skills:

• Active New York State LPN License
• Current BLS Certification
• Minimum 2 years of LPN experience in Long-Term Care (LTC), Skilled Nursing Facility (SNF), or related setting
• Strong clinical assessment and documentation skills
• Ability to work every other weekend
• Excellent communication and teamwork abilities

Preferred

• Previous Skilled Nursing Facility (SNF) experience
• Experience with geriatric and long-term care residents
• Familiarity with electronic medical records (EMR)
