LVN / LPN – Hospice Patient Care Advocate (Referral Outreach)
Location: Marin County, CA (field-based)
Role Summary
This field-based LVN/LPN supports hospice referral growth by partnering with hospice liaisons, educating referral sources and families, and helping guide appropriate patients through the hospice admission process.
You'll combine clinical knowledge with strong relationship-building and communication skills to explain hospice services, support admissions, and document outreach activity in a CRM.
Schedule: Monday–Friday, 8:00am–4:30pm
Territory: Marin County
Key Responsibilities
- Partner with hospice liaisons to identify and develop referral sources across the territory.
- Educate clinicians, facilities, patients, and families on hospice eligibility, services, and benefits of timely referral.
- Gather key clinical information to support admissions and ensure smooth handoffs to the intake/clinical team.
- Participate in community and facility outreach activities to build awareness and relationships.
- Document outreach, referral activity, and follow-ups accurately in a CRM; provide regular activity reporting.
Requirements
- Active, unencumbered LVN or LPN license (California).
- 1+ year hospice clinical experience (2–3 years preferred).
- Strong communication, consultative education, and relationship-building skills.
- Comfortable with outreach, marketing activity, and working toward referral/census goals.
- Proficient with CRM/documentation and basic reporting.
- Valid driver's license, reliable insured vehicle, and ability to travel throughout the territory.
- Preferred: prior sales, liaison, admissions, or customer-facing healthcare experience.
