LVN Job Description
Provide direct patient care to residents in a Skilled Nursing Facility
Administer medications and treatments as prescribed by physicians
Monitor and document patient conditions and progress
Assist with assessments and development of individualized care plans
Collaborate with Registered Nurses, physicians, therapists, and support staff
Ensure compliance with facility policies, state regulations, and quality standards
Respond to changes in patient conditions and communicate updates to the healthcare team
Educate residents and family members regarding care plans and treatments
Maintain accurate and timely patient records
Support a positive, compassionate, and patient-centered environment
Education & Certification Requirements
Current California Licensed Vocational Nurse (LVN) license in good standing
Graduate of an accredited Practical Nursing or Vocational Nursing program
Current BLS (Basic Life Support) certification required
Minimum of 2 years of experience in a skilled nursing facility or acute care setting
Strong clinical assessment and communication skills
Ability to work independently and as part of a collaborative healthcare team
