At Cornerstone VNA, we believe exceptional care happens where patients feel most comfortable-at home. We're looking for a compassionate Physical Therapist (PT) or Occupational Therapist (OT) with lymphedema training to join our growing Home Health Rehabilitation team.

This is a unique opportunity to build and grow a specialized lymphedema program while making a meaningful difference in the lives of patients throughout the Seacoast.

Whether you're looking to dedicate your practice entirely to lymphedema care or prefer a combination of lymphedema and traditional home health therapy, we offer the flexibility to build a schedule that fits your interests and expertise.

What You'll Do

As our Lymphedema Therapist, you'll provide comprehensive evaluation and treatment to patients in their homes while collaborating with an experienced interdisciplinary team.

Responsibilities include:

• Evaluate patients with lymphedema and develop individualized treatment plans.
• Provide evidence-based lymphedema treatment in the home setting.
• Educate patients and caregivers on self-management techniques and long-term care.
• Assess, recommend, and coordinate compression garments and pneumatic compression pumps.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, and rehabilitation staff to optimize patient outcomes.
• Participate in Lymphedema Team meetings every other month.
• Maintain accurate and timely electronic documentation.
• If desired, maintain a mixed caseload of both lymphedema and traditional home health therapy patients.

Qualifications

• Current Physical Therapist (PT) or Occupational Therapist (OT) license in New Hampshire and Maine
• Certification or specialized training in lymphedema treatment (CLT preferred).
• Current driver's license, reliable transportation, and automobile insurance.
• Excellent communication and clinical reasoning skills.
• Home Health experience is preferred but not required.
