The AMS pharmacist provides direct patient care including management, monitoring, and instruction related to anticoagulation management. The pharmacist will provide high quality patient care through a collaborative practice agreement for anticoagulation management of patients referred by any Carle Provider to AMS. They will be responsible for implementation, monitoring, and overall management of anticoagulation therapy regimens. They will provide direct supervision and instruction to AMS pharmacy technicians. Certifications: Licensed Pharmacist - Illinois Department of Financial and Professional Regulation (IDFPR), Education: Bachelor's Degree: Pharmacy, Work Experience: Work in collaboration with other healthcare professionals to provide safe, effective anticoagulation management. Represent the pharmacy on committees or work groups as assigned. Assist in the training and orientation of new employees as assigned. Provide Medication Therapy Management services as defined in the Illinois Pharmacy Practice Act Maintains current knowledge of and ensures compliance with government and commercial regulations. Call and triage patients with abnormal INR lab results, assess clinical status, and determine appropriate next steps. Manage patient's warfarin per protocol or collaborative practice agreement. assist in transitions of care, including coordination of hospital discharges with the inpatient pharmacy team. Manage excellent patient communications including telephone messages/patient encounters for questions, triage, and education.
