Job Details:

 As a Manager of Care Coordination, you will discuss discharge needs with Physicians and Social Workers; and partner with unit nurses to communicate with patients/family about admission and plan of care. 

 

Qualifications:

• Active NY Registered Nurse license
• 2-3 years experience in health care related discipline and/or discharge planning or community healthcare experience
• BSN preferred
• Excellent interpersonal skills
• Ability to respond to short deadlines

 

 

Posted Salary Range: USD $50.64 - USD $73.44 /Hr. :
