Title: Manager – Clinical Resource / Case Management (RN) – Contract

Location: Hyannis, Massachusetts

Duration: Contract (Approx. 13 Weeks)


Position Overview

"Navitas Healthcare, LLC" is seeking an experienced RN Leader for a Clinical Resource / Case Management Management role in an acute care hospital setting. This position oversees Case Management, Utilization Review, Social Work, and Clinical Documentation teams, ensuring effective discharge planning, care coordination, and regulatory compliance.


Key Responsibilities

  • Manage daily operations of Case Management, Social Work, and Clinical Resource teams
  • Oversee staffing schedules, workflows, and performance evaluations
  • Lead discharge planning, utilization review, and high-risk patient coordination processes
  • Conduct regular patient unit rounds to support discharge efficiency
  • Develop and deliver training and education programs for CM/SW staff and clinical teams
  • Monitor compliance metrics and create performance dashboards
  • Identify process gaps and implement quality improvement initiatives
  • Collaborate with physicians, nurses, and interdisciplinary teams
  • Stay current with CMS regulations and case management standards
  • Promote continuous improvement and clinical best practices

Required Qualifications

  • Active Massachusetts RN License required
  • Bachelor’s in Nursing required (Master’s preferred)
  • Minimum 5 years acute care case management experience
  • Minimum 3 years acute care clinical experience
  • Experience in utilization management, discharge planning, and care coordination
  • Knowledge of CMS regulations
  • Strong leadership, communication, and analytical skills
  • Experience with InterQual or similar utilization review tools preferred
  • Prior supervisory or management experience preferred
  • Experience developing and delivering training programs preferred
