Job Summary:

Responsible for ensuring Diagnostic Imaging department quality metrics and safety goals are achieved and maintained. Contributes to Quality and Safety goals and objectives for the department that align with the strategic direction of the organization by analyzing needs and developing recommendations. Communicates goals and objectives to unit managers and staff, and monitors progress during the year with demonstrable updated monthly metrics which are then reported to the appropriate areas and individuals. Member of multiple committees across the hospital related to quality and safety, representing Diagnostic Imaging.

Job Responsibilities:

Committee Responsibilities:

• Member of the weekly HH Root Cause Analysis committee to discuss and review adverse events that occur across the hospital. Develops detailed timelines for safety events that may require RCA's in conjunction with the hospital Q&S team.
• Belongs to the HH Continuing Readiness Committee that meets once per month in order to stay current with DPH and Joint Commission standards. Reviews, adopts and implements new rules and regulations from all accrediting bodies.
• Sits in on the Environment of Care committee to assure that all regulatory changes and updates are disseminated to the radiology units.
• Belongs to the Clinical Quality Council which updates members with KPI reports, CMS ratings and other various quality events that occur. May be asked to present RCA's that pertain to Radiology.
• Participate in select workgroups that evolve to meet the needs to the system. Collaborate with Quality and Safety team members from other departments on projects that align with HHC's commitment to patient safety and excellence. (Including infection Control champ, Safety Champs, specimen labeling error group, PolicyTech workgroup, Human experience committee, etc.)
• Presents annually to the Quality and Process Improvement Council (QAPIC) with reports on initiatives, metrics and opportunities/barriers in attaining forward progression for projects.
• Participate in the ED Q&S council and report specific radiology based metrics to this council.
• Schedule and attend IR and other departments' Mortality & Morbidity reviews to keep current with each case. Apply for CMEs for each M&M.

Metrics:

• Run monthly metrics for the system wide radiology dashboard as well as ED dashboard, report these to the Radiology Chair and Director as well as in the monthly Q&S meeting.
• Work with Residents to attain metrics, data and reports for their various PQI projects. Monitor and coach their end of CT to read times for all strokes to ensure a fast turn-around time.
• Review all CT Chest cases for PE presence regularly and report this to the VTE coordinators.
• Work closely with Stroke Coordinator and Stroke time to keep real time updates on Joint Commission reportable metrics. When cases do not make target time the Q&S manager will create awareness and education to all applicable team members to decipher where the process had a gap. Will report out monthly to the Neuro Council with multiple metrics regarding JC reportable stroke metrics and concerns.
• Maintain a public database of all radiation emitting equipment in the department and ensure that a HH approved physicist keeps timely surveys of each piece. Monitor ultrasound and MRI equipment and those surveys.
• Document and report on PPE intake and distribution patterns in conjunction with the radiology stockroom.
• Creates and distributes monthly 'Exposure' newsletter to the department of radiology
• Reviews and documents monthly updates of current credentialing/licensure status for all staff and radiologists. add the credentials of new radiologists to our records

Develop and implement corrective action reports regarding events that are deemed reportable to the state. Follow through on audits that follow each reportable event.

Work closely with radiologists as well as Director and Managers of Diagnostic Imaging on events that need second reads and follow ups.

Rounds and audits to ensure that the required Joint Commission, DEEP (DPH) and MQSA are being met by supervising and auditing all areas of Radiology in order to maintain the hospital's accredited status.

Monitors policies and policy renewals, and provides edits as needed. Sits on the Radiology Council Subcommittee to develop new policies in conjunction with other Quality & Safety team members across the system. Work with Radiation Safety Councils, MEC and other committees to guide policies through the proper channels to be vetted.

Chair the monthly Quality and Safety meeting for Radiology. Items include Environment of care, accrediting body surveys, audits, metrics, PPE information, infection control practices or changes, benchmarks, and current initiatives.

Daily review and update for Quantros incident reporting. This includes closing out and assigning events to particular departments. Member of weekly Patient Safety Event Review Team lead by HH Quality and Safety team.

Qualifications

Requirements and Specifications

Education

• Bachelor''s degree in Radiology, Business Management, Health Science, or related field required.

Experience

• Minimum 5 years in Radiology with clinical experience

Licensure, Certification, Registration

• Current certification or registration in a Diagnostic Imaging Modality and Current State of Connecticut Radiologic Technologist Licensure

Knowledge, Skills and Ability Requirements

• Experience with EPIC, PACS and digital technology

• Knowledge of continuous improvement and project management

• Excellent customer service skills with a professional attitude.

• Strong computer skills for use with email, Word and Excel.

• Strong troubleshooting and problem solving skills

• Knowledge of Radiological principal and practices. Sufficient to be able to perform a wide range of duties relative to the work assigned.

• Ability to lead and make sound decisions.

• Ability to multitask and prioritize.

• Experience working with healthcare associated accrediting bodies
