POSITION SUMMARY

The Manager of Care Coordination (RN) is responsible for the leadership, development, implementation, and oversight of KC CARE's Care Coordination and Nursing Programs. This position provides supervision of Patient Health Coordinators (PHCs), Care Coordinators, and assigned nursing staff while ensuring patients receive comprehensive, patient-centered care that addresses medical, behavioral health, social, and community-based needs. Serving as the operational leader for Population Health nursing and care coordination services, the Manager provides clinical oversight, workflow development, quality monitoring, and staff development. In partnership with the Vice President of Population Health, the Manager supports population health strategies, value-based care initiatives, quality improvement efforts, and compliance with organizational, grant, payer, and regulatory requirements. The Manager also serves as a clinical resource for complex patient needs, fosters interdisciplinary collaboration, and leads initiatives that improve patient access, care transitions, chronic disease management, and overall health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Ensures compliance with and supports organizational performance for Patient-Centered Medical Home (PCMH), Primary Care Health Home (PCHH), HRSA, Medicaid, value-based care, grant-funded initiatives, and other applicable federal, state, and payer programs through effective care coordination, care management, quality improvement, and population health activities
  • Provides direct supervision and leadership of Population Health nursing staff and nursing programs.
  • Oversees nursing workflows related to care management, chronic disease management, transitions of care, preventive services, and population health initiatives.
  • Ensures nursing programs operate in accordance with evidence-based practice, organizational policies, and regulatory requirements.
  • Partners directly with the Vice President of Population Health to establish departmental goals, strategic priorities, quality initiatives, and workforce planning.
  • Provides coaching, mentoring, and professional development opportunities for nursing staff.
  • Monitors nursing productivity, quality measures, patient outcomes, and program effectiveness.
  • Participates in workforce planning, recruitment, retention, and succession planning for nursing and care coordination staff.
  • Oversees nurse-led care management programs for high-risk, medically complex, and vulnerable patient populations.
  • Leads nursing initiatives focused on chronic disease management, preventive care, care transitions, and population health outcomes.
  • Collaborates with clinical leadership to develop protocols and workflows that support team-based care and improve quality performance.
  • Ensures appropriate coordination between nursing, medical, behavioral health, dental, pharmacy, and community-based services.
  • Utilizes population health data and quality metrics to identify gaps in care and implement interventions to improve outcomes.
  • Oversees quality monitoring and performance improvement activities for nursing and care coordination programs.
  • Tracks and reports key nursing and population health metrics, including care management outcomes, transitions of care, preventive services, and quality measures.
  • Collaborates with the Vice President of Population Health and Quality Department to develop strategies that improve clinical outcomes and patient experience.
  • Provides leadership and oversight of the Patient Health Coordinator (PHC) program through direct supervision of the PHC Supervisor.
  • Collaborates with the PHC Supervisor to establish program goals, monitor performance metrics, and ensure effective patient outreach, navigation, and care coordination activities.
  • Reviews program outcomes and identifies opportunities to improve patient engagement, care gap closure, referral completion, and access to services.
  • Supports the development and implementation of workflows, policies, and training related to patient navigation, community resource linkage, and social determinants of health.
  • Serves as an escalation resource for complex patient situations and barriers to care that require interdisciplinary collaboration.
  • Partners with the Vice President of Population Health to evaluate program effectiveness and align PHC activities with organizational quality, population health, and value-based care initiatives.

MINIMUM REQUIREMENTS

  • B.S.N. in Nursing
  • Licensed as a Registered Nurse in the State of Missouri
  • 5 years' experience in internal medicine/family medicine/community health
  • 2 years' progressive management experience, including program planning, evaluation and budget responsibilities.

PREFERRED REQUIREMENTS

  • 10 years of clinical experience, with at least 5 years in leadership or supervisory role.
  • Strong knowledge of healthcare regulations, quality assurance, and clinical best practices.
  • M.S.N in nursing or related field
  • Experience working in a community health or FQHC setting.
  • Experience in Federal grant management.

Working Conditions

General working conditions are in a healthcare facility. As such, there is a potential exposure to virus, disease, and infection from patients while performing the duties of this job. May experience traumatic situations including psychiatric traumatized and deceased patients. Travel may be expected to community meetings, client homes, or other agencies.
