Apply Today!Job Summary and QualificationsAs Manager for Case Management, you will report to the Case Management Director and will be responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management within your assigned area of responsibility in alignment with the goals of the Case Management DepartmentYou will provide day-to-day oversight and management of the case management program, ensuring effective coordination, supervision, and administrative support of the case management teamActing as a case management subject-matter expert, you will serve as a resource, advisor and leader for the members of the case management teamYou will supervise and monitor professional and support staff, and ensure effective care coordination and case management practices consistent with hospital policies, applicable regulations and guidelinesYou will evaluate the quality of interactions, documentation and assessments conducted by the case management staff and facilitate initiatives that promote quality outcomes and improve patient throughput while supporting a balance of optimal care and appropriate resource utilizationWhat qualifications you will need:Bachelors in Nursing OR Masters in Social Work required5 years of case management in an acute hospital setting requiredThorough knowledge of Case Management defined by established rules and regulations of federal and non-federal programs, and understanding of hospital reimbursement as it relates to different payers such as Medicare, Medi-cal, Managed Care and private insurance plansFounded in 1901, Riverside Community Hospital is a 542-bed full-service acute care hospital.
