About the Clinical Manager Role
This leadership role is ideal for a dedicated clinical professional who thrives in a collaborative environment and is passionate about improving patient care. The position oversees clinical support staff, ensuring high-quality care delivery through chronic and transitional care management initiatives. With a strong focus on workflow efficiency, data-driven decision-making, and cross-functional collaboration, this role plays a key part in optimizing patient outcomes. The ideal candidate will bring experience in healthcare leadership, staff development, and telehealth, along with a commitment to fostering a supportive and high-performing clinical team.

What you'll do:

• Collaboration and Care Guidelines Development
• • Collaborate with other clinical leaders to develop care guidelines, pathways, and criteria aimed at improving patient interactions and achieving better quality outcomes.
  • Provide expert insight on delivering care in patients' homes and through telehealth platforms.
• Team Leadership and Staff Management
• • Provide leadership and guidance to clinical support staff, including RNs and Care Navigators, to ensure they perform their duties effectively and deliver high-quality care.
  • Oversee recruitment, hiring, and management of the clinical support team.
  • Ensure the successful onboarding of new staff members, providing the necessary training and support.
• Chronic and Transitional Care Management
• • Management of chronic care management and transitional care management initiatives, including outreach, assessment, and intervention for patients.
  • Monitor and evaluate the effectiveness of care management strategies and make recommendations for continuous improvement.
• Performance Monitoring and Staff Development
• • Monitor clinical staff performance, conduct regular performance evaluations, and provide constructive feedback.
  • Address performance issues in a timely and supportive manner and promote continuous professional development for team members.
  • Hold monthly one-on-one meetings with staff members to discuss performance, provide feedback, and address concerns.
• Data-Driven Decision Making
• • Use data from Key Performance Indicators (KPIs) to make informed decisions and drive improvement initiatives.
  • Review outcome data and participate in performance improvement activities to enhance patient outcomes.
• Workflow Efficiency and Cross-functional Collaboration
• • Work cross-functionally with other departments to design and implement workflow efficiency initiatives.
  • Ensure that clinical processes are streamlined and aligned with organizational goals to improve operational effectiveness.
• Administrative Oversight
• • Approve Paid Time Off (PTO) requests and oversee time clock management to ensure proper staffing and operational continuity.
  • Hold weekly clinical meetings to discuss updates and review best practices.
  • Perform various clinical and administrative tasks as needed, ensuring the smooth operation of the clinical department.

Required Skills & Competencies:

• Licensing/Certifications: Active Registered Nurse (RN) license or other clinical licensure relevant to the role; Additional certifications in healthcare management or clinical care (preferred).
• Experience: Minimum of 3-5 years of clinical experience in a healthcare setting, with at least 2-3 years in a leadership or supervisory role.
• • Experience with chronic care management and transitional care management.
  • Familiar with telehealth care delivery and home-based patient care.
• Skills:
• • Strong leadership, communication, and interpersonal skills.
  • Data analysis and performance management expertise.
  • Ability to foster a collaborative and team-oriented environment.
  • Proficiency in healthcare software and Electronic Health Records (EHR) systems.
  • Knowledge of healthcare regulations, quality standards, and compliance requirements.

Working Conditions:

• This position may require occasional evening and weekend work based on staffing needs and team support.

Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
