About the Role:
We're seeking an energetic, organized leader with experience in community engagement and social work or case management to manage a team of Community Health Workers serving hundreds of clients annually. This position is offered through a partnership between Highmark Health in Pittsburgh, PA and IMPaCT Care. While the role supports Highmark Health's community health initiatives, you will be directly employed by IMPaCT Care.
