Job Summary and Qualifications
Under the supervision of the Director of Pharmacy, the Manager of Pharmacy Services is responsible for designing, implementing and managing the pharmacy clinical programs. This includes formulary management, policy and procedure development and education across the healthcare spectrum. The Manager shall also assist in managing the pharmacy when the Pharmacy Director or designee is unavailable.
Primary Responsibilities:
- You will Identify, design and analyze drug utilization reviews to promote rational drug therapy and determine action plan based on results.
- You will develop, manage and mentor pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as determined appropriate based on patient population and literature guidelines.
- You will develop, manage and mentor antimicrobial management program (AMP).
- You will be responsible for design, analysis and reporting of adverse drug reactions, medication errors and pharmacy quality initiatives and mentor others in their roles for these programs
- You will maintain appropriate controlled substance records and work with the Pharmacy Director to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards.
- You will comply with all regulatory standards and work with the Pharmacy Director and Operations Manager to identify and develop processes to meet or exceed all regulatory standards.
- You will participate in quality and regulatory reviews and audits.
- You will work with the Director to establish pharmacy goals and objectives and responsible for implementation and monitoring.
- You will be responsible for managing and monitoring the drug formulary,
- You will be responsible for the development, implementation, and monitoring of pharmacy clinical initiatives and share pharmacy operation responsibility with Pharmacy Operations Manager.
- You will be responsible for the development and review of pharmacy policies and procedures.
- You will interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.
- You will accurately compound and dispense pharmaceuticals.
- You will comply with facility policy and procedures and regulatory requirements.
- You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
- You will provide clinical consultation and clarification to practitioners and suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.
- You will support participation in daily multidisciplinary and bedside rounds. (you may need to attend).
- You will provide accurate and timely medication information to healthcare staff and patients.
- You will document all clinical activities and interventions accurately and completely.
- You will develop and manage specialty pharmacy practice programs related to clinical pharmacy services.
- You will provide basic technical support for automated dispensing machines and other pharmacy computer systems.
- You will participate with various nursing and medical staff committees as necessary.
- You will be responsible for development and review of physician order sets and other evidenced based medication use guidelines.
- You will be responsible for routine publication of the pharmacy newsletter and/or other forms of written and electronic communication.
- You will be responsible for drug class reviews and drug monographs.
- You will participate in the design and update of pharmacy computer system programs relating to pharmacy, nursing and physician programs.
- You will participate in the maintenance of pharmacy dictionaries including adding new drugs to the drug dictionary and Gold Standard formulary Charge Master maintenance.
What qualifications you will need:
EDUCATION:
- Graduate from an ACPE accredited college of pharmacy with a Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (Pharm D) degree.
EXPERIENCE:
Minimum of 1 year as a Pharmacy Clinical/Operations Specialist or one of the following:
- - At least 5 years of hospital pharmacy experience; preferably within area of focus.
- - Completion of hospital PGY1/pharmacy specialty residency or fellowship.
- - Other certification (e.g. FCCP, FASHP).
- - Hospital pharmacy management experience.
- - Other certifications (e.g. FCCP, FASHP) preferred.
- - Hospital pharmacy management experience preferred.
LICENSURE, CERTIFICATIONS:
- Registered Pharmacist with the state of Texas required.
