    The Manager, Population Health & Value-Based Care provides operational and analytical leadership for Terros Health's population health programs, value-based care performance, Targeted Incentive Plan (TIP), and proactive care gap closure strategies. Reporting to the Vice President of Quality, this role oversees multidisciplinary functions responsible for risk stratification, value-based care program development, implementation and monitoring, and payer/regulator driven quality programs, and other diverse population level performance improvement. The Manager ensures that population health and value-based initiatives are aligned with organizational goals, payer expectations, reimbursement opportunities, and integrated care models across all Terros Health service lines.

    Population Health Strategy & Program Leadership

    Value-Based Care Performance & Payer Program Management

    • Manages all value-based care (VBC) and TIP program activities, including attribution, measure tracking, incentive reporting, and payer collaboration.
    • Monitors performance on VBC, TIP, and payer specific quality measures; develop action plans to close gaps and improve outcomes.
    • Serves as the liaison with internal and external partners

    Value-Based Care Performance & Payer Program Management

    • Manages all value-based care (VBC) and TIP program activities, including attribution, measure tracking, incentive reporting, and payer collaboration.
    • Monitors performance on VBC, TIP, and payer specific quality measures; develop action plans to close gaps and improve outcomes.
    • Serves as the liaison with internal and external partners

    Data Driven Performance Improvement

    • Collaborates with Business Intelligence, Quality, and Clinical Operations to develop dashboards, registries, and actionable analytics.
    • Translates data into operational strategies that improve outcomes for screenings, SDOH, chronic disease, and preventive care.
    • Leads monthly performance reviews with clinics and service line leaders.

    SDOH, Screening, and Preventive Care Initiatives

    • Oversees SDOH screening workflows, referrals, and follow up processes.
    • Ensures consistent implementation of preventive screenings (depression, HIV, cancer screenings, etc.) across all sites.
    • Partners with community organizations to support social needs navigation.

    Leadership & Supervision

    • Provides direct leadership, coaching, and mentorship to team members to foster professional growth, excellence, and strong service delivery.
    • Establishes performance expectations, conducts evaluations, and supports goal-setting for all supervised staff.
    • Builds and maintains a collaborative, high-performing team through effective communication, training, and continuous improvement.
    • Oversees workload distribution, resource planning, and staffing recommendations to ensure operational efficiency.
    • Promotes a culture of accountability, customer service, and innovation across the organization.

    • Bachelor's degree in Public Health, Healthcare Administration, Nursing, Health Informatics, Business Administration, Social Work, or related field required.

    • A minimum of 5 years of progressive experience in population health management, quality improvement, value-based care, healthcare operations, or related program management, including at least 2 years of leadership or supervisory experience.

    • Experience working in a Federally Qualified Health Center (FQHC), integrated healthcare, behavioral health, primary care, Medicaid managed care, AHCCCS, or similar healthcare setting strongly preferred.

    • Experience working with payer incentive programs, alternative payment models, Targeted Incentive Program (TIP) initiatives, or other value-based contracting arrangements preferred.

    • Knowledge of healthcare quality measures, risk stratification methodologies, panel management, preventive care initiatives, chronic disease management, and social determinants of health (SDOH) programs.

    • Demonstrated experience managing population health initiatives, care gap closure strategies, quality improvement programs, and/or value-based reimbursement models.

    • Understanding of regulatory, payer, and quality reporting requirements impacting population health and value-based care programs.

    • Strong analytical and problem-solving skills with experience utilizing healthcare data, dashboards, registries, and performance reporting tools to guide operational and clinical decision-making.

    • Experience interpreting and presenting data to executive, operational, and clinical leaders to support strategic planning and performance improvement efforts.

    • Demonstrated ability to lead teams, manage complex workflows, and drive performance improvement

    • Must have a valid Arizona driver's license, be 21 years of age with a minimum of 4 years driving experience, and meet requirements of Terros Health's driving policy

    • Must successfully pass a TB screening and criminal background check.
