    Description

    The Manager of Population Health programs provides strategic direction and operational oversight for various quality and value-based programs managed through Pacific Medical Centers. Responsibilities include population health, value/risk-based program that fosters, promotes and is dependent on integration of services and collaboration across several acute, non-acute and health plan ministries.

    Required Qualifications:

    • Bachelor''s Degree Healthcare Administration, Public Health or related field.
    • 2 years Progressive health care leadership experience that includes: Healthcare services development, delivery, and operations.
    • 2 years Experience leading interdisciplinary care teams, and planning complex geriatric and long term care.

    Preferred Qualifications:

    • Master''s Degree Healthcare Administration, Public Health or related field.
    • 2 years Experience developing and executing successful strategic business plans.
    • 2 years Experience supervising staff in a matrix structured organization.
    • 2 years Experience with Project Management methodologies and software such as CAP, Work-Out and Lean.

    Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate''s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
