Position Overview:
Manage the overall operations of the department in accordance with current federal and state regulations and guidelines. Provides operational leadership for an integrated social services program, ensuring the psychosocial, emotional and medical needs of patients and/or a targeted population group are addressed to support optimal health and best clinical outcomes. Collaborates with the interdisciplinary team to facilitate transitions/care across the continuum, providing appropriate linkages to community resources to support care and to achieve desired clinical/treatment outcomes. Ensures cost effective utilization of resources to support the achievement and/or improvement of dashboard measures and other key strategic initiatives.

Pay Range is $157,830.40 to $236,745.60 / annual salary
The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate's experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health's comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
