Position: Market Manager Monogram Health is seeking a leader who will be responsible driving performance in their assigned market. The market performance manager will lead a team of nurse practitioners, registered nurses, social workers, and support team members to care for and improve health outcomes of an attributed patient panel. This individual will be accountable for the overall clinical and operational performance of their assigned market. The market manager will be charged with achieving company metrics including but not limited to admissions, blood pressure and diabetes management, conservative pathways execution, dialysis starts, and staff productivity. The market performance manager will work closely with clinical and corporate leaders to ensure exceptional patient care is being provided by their team. He/she should be a champion of change and embrace opportunities to improve performance. Experience managing a high-risk patient population and leading clinical staff in a care management, managed care and/or or home health environment are preferred. Microsoft Excel and Salesforce experience are also preferred. Must have experience analyzing clinical and operational performance. Roles and Responsibilities Manage performance against clinical and operational metricsLead, motivate, and drive accountability for a team of nurse practitioners, registered nurses, and social workersPrepare, review and analyze performance and health outcome reports to identify areas requiring attention and improvement and acting quickly to resolve known issuesPlan staffing schedules to optimize efficiency and productivityAssist in the hiring of clinical staff membersExecute corporate and regional strategy to deliver quality patient carePerform other duties as assignedUphold the mission and values of Monogram Health in all respectsBilingual PreferredPosition Requirements This position is remote but may require 25% of travel within their given marketBasic Life Support (BLS) certification is required in this role.
