Job Summary
A Grant Services Specialist II provides a wide variety of grant-funded paraprofessional administrative, programmatic and complex general support services work to an assigned City agency or program.
Work of this class involves no supervisory duties or responsibilities. Incumbents receive general supervision from an administrative superior. Employees in this class work a conventional workweek that may include evening and weekend hours. Work is generally performed in an office or clinic where working conditions are normal. Work generally requires minimal physical exertion.
The Dementia Navigation Program Assistant will provide administrative, programmatic, and community outreach support for the Dementia Navigation Program. The role is responsible for assisting with the coordination of educational programs, outreach events, and community engagement activities while serving as one point of contact for individuals living with dementia, their caregivers, and community partners. The Dementia Navigation Program Assistant helps connect callers to appropriate resources and supports the successful delivery of dementia- related services and initiatives. The Dementia Navigation Program Assistant will be crossed trained to assist the Maryland Access Point Call Center when needed.
Essential Functions
Minimum Qualifications
Education: Have a bachelor's degree from an accredited college or university.
AND
Experience: One year of experience in coordinating, monitoring and processing cases involving the provision of health, social, vocational or rehabilitation services to clients or equivalent combination of education and experience.
OR
Equivalency Notes: Have an equivalent combination of education and experience.
Preferred Qualifications:
Education: Associate degree in human services, social work, public health, healthcare administration, or a related field preferred; equivalent experience and/or education will be considered.
Experience: Two years of experience in administrative support, community outreach, healthcare, aging services, call center experience, or a related field preferred

Probation
