Provide a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, crisis intervention, group facilitation, and/or case management services in an integrated, multi-disciplinary primary care setting, located in Central MA. Work collaboratively with other mental health and medical providers and trainees engaged in related therapeutic/patient care activities.
Primary Responsibilities
- Provides individual, group, and family therapy to clients as appropriate to the position; prepares treatment plans, and case management including discharge planning; provides therapeutic crisis stabilization services as required.
- Maintains relationships with referral partners; collaborate with outside treatment providers agencies in relation to patient/client records, rights, and responsibilities while engaged in day treatment.
Requirements
- Master's degree in Counseling or Social Work.
- Licensed Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC) or equivalent.
